    Altered right portal and umbilical vein doppler parameters in fetal macrosomia resulting from pregestational and gestational diabetic mothers: A prospective case-control study

    Objective: To evaluate the blood flow of the umbilical vein and right portal vein in macrosomic fetuses of diabetic mothers and investigate the effect of maternal insulin treatment on these blood flows.Material and Methods: This prospective case-control study was con- ducted between March 2019 and December 2019. Fetuses of the 49 pregestational and gestational diabetic mothers who had an abdominal cir- cumference percentile above 97% were evaluated as macrosomic and formed the study group. The study group was divided into two subgroups: patients treated with insulin and those who did not. In the control group, 48 non-diabetic pregnant women with matched gestatio nal weeks whose fetuses are at the 10-90% percentile were included. Time-averaged maximum blood velocity (TAMXV) values of the right portal vein and the free loop of the umbilical vein were measured.Results: The median right portal vein TAMXV value and umbilical vein TAMXV value were found to be significantly higher in diabetic pregnancies (16.25 cm/s, and 15.28 cm/s, respectively) than in the control group (12.76 cm/s, and 13.38 cm/s, respectively, p<0.001). Umbilical and right portal vein flows were similar in macrosomic fetuses of diabetic mothers who were treated with insulin or those who did not. While umbilical vein flow in macrosomic fetuses increased as the gestational age pro gressed (p=0.028), it was observed steadily in normally growing fetuses. Conclusion: The umbilical and right portal vein flows are higher in macro- somic fetuses of diabetic mothers than in appropriately grown fetuses. Maternal insulin treatment does not affect fetal umbilic al vein and right portal vein blood flow in macrosomic fetuses

    The Effect of Nasal Functions on the Integrity of Grafts after Myringoplasty

    Objective:We aimed to evaluate the effects of nasal functions for the integrity of grafts after myringoplasty.Methods:In our study 78 patients who underwent myringoplasty operation between 2011-2013 were included. Group I was defined as the group with an intact tympanic membrane following surgery. Group II was defined as the group with a tympanic membrane perforation following surgery. Group I consisted of 44 and Group II consisted of 34 patients. Subjective and objective measurements of nasal functions, Eustachian tube function (ETF), and allergic status were performed using nasal obstruction symptom evaluation (NOSE) scale, visual analog scale (VAS), and the score for allergic rhinitis (SFAR) questionnaires and acoustic rhinometry and saccharin test. It was investigated whether there was any difference between these two groups in terms of these parameters.Results:There was statistically no significant difference between groups according to the age, sex and the presence of tubal dysfunction and allergic rhinitis (p>0.05). In the group of intact tympanic membranes, the likelihood of right ear being the operated one was significantly higher compared to the group of myringoplasty failures (p=0.037). The VAS and NOSE scales did not show any significant difference between groups in terms of successful outcome of myringoplasty (p>0.05). The nasal congestion index (NCI) and the mucociliary clearance (MCC) did not show any significant difference between groups in terms of successful outcome of myringoplasty (p>0.05).Conclusion:This study has shown that nasal functions measured by objective and subjective methods had no effects on the success of myringoplasty

    Effects of topical mometasone furoate therapy on local and systemic macrophage migration inhibitory factor levels in allergic rhinitis patients

    Amaç: Bu çalışmada mometazon furoat nazal sprey (MFNS) ile tedavi ettiğimiz alerjik rinitli hastalarda tedavi öncesi (TÖ) / tedavi sonrası (TS) serumda ve nazal lavaj sıvılarında makrofaj migrasyon inhibitör faktör (MİF) düzeylerini araştırdık. Ayrıca, tedavinin naza lmukozadaki eozinofil düzeyine etkilerini irdeledik. Gereç ve Yöntem: İlk muayene sırasında alerjik rinit yakınmalarının en az ikisine sahip olan 22 hastanın semptomları skorlandı, muayene bulguları kaydedildi; serumda ve nazal lavaj sıvısında MİF değerleri saptandı. Hastalara günde bir kez 100 g MFNS (Nasonex®) başlandı. TS dördüncü haftada semptomlar ve muayene bulguları yeniden değerlendirildi, nazal lavaj/serum MİF değerleri ELISA metodu ile ölçüldü. Nazal mukoza impresyonsitolojisimateryallerinde eozinofillerin diğer hücrelere göre oranları hesaplandı. Bulgular: TÖ/TS nazal lavaj MİF ortalaması (sırasıyla 503,53 294,50 pg/ml ve 1422.35 1097,98 pg/ml, P0.05) bulundu. Nazal mukozanın impresyon sitolojisinde eozinofil yüzdesi TÖve TS, sırasıyla 51.86 28,94 ve 25.64 23,60 idi. Eozinofil yüzdeleri arasındaki fark istatistiksel olarak anlamlıydı (p<0.001). Semptomskorları ortalaması TÖve TS sırasıyla 16,95 4,27 ve 9,24 3,38 idi. Skor ortalamalarındaki fark istatistiksel olarak anlamlıydı (P<0.001). Sonuç: HastalarımızdaTSda anlamlı klinik bir düzelme görüldü.TS semptomskorları ve nazalmukoza eozinofil yüzdeleri belirgin şekilde azaldı. TSda lokal MİF düzeylerinde ise artış izlendi. MFNS tedavisini takiben MİF değerlerinde izlenen anlamlı değişikliğin klinik düzelmeyle ilişkisinin daha ayrıntılı incelenmesi gerekmektedir.Background: In this study we aimed to investigate the systemic and local levels of macrophagemigration inhibitory factor (MIF) before and after topically applied mometasone furoate therapy in patients with allergic rhinitis. We also investigated the effects of therapy on local eosinophil accumulation. Material and Methods: Twenty-two patients (8 males and 14 females; median of 46 years ranging, from 17 to 68 years) with AR were subjects of this study. Percentages of eosinophils in nasal smears were calculated. MIF levels in serum and nasal lavage were measured by an enzyme-linked immunosorbent assay. Results: Topical glucocorticoid treatment significantly inhibited total symptom score (TSS) (16,95 4,27 and 9,24 3,38, respectively, p<0.001). After therapy, mean percentages of eosinophils decreased significantly (51.86 28,94 and 25.64 23,60 respectively, p<0.001) by impression cytology. MIF levels both in serum (782,55 759,43pg/ml and 917.10 1080,37 pg/ml, respectively) and nasal lavage (503,53 294,50 pg/ml and 1422.35 1097,98 pg/ml, respectively) increased after mometasone therapy. However, only the change in nasal lavage samples was statistically significant (p<0.001). Conclusions: TSS and nasal eosionophil numbers in nasal mucosa decreased following therapy. Further investigations are needed to evaluate the relations between clinical improvement and the significant changes in MIF levels following MFNS treatment

    Ultrasound processing of vinegar: Modelling the impact on bioactives and other quality factors

    In recent years, non-thermal technology has been used for the enrichment of ultrasound bioactive components. For this purpose, it was applied to tomato vinegar and modeled with response surface methodology (RSM) and artificial neural network (ANN). At the end of the RSM, cupric reducing antioxidant capacity (68.64%), 1,1-diphenyl-2-picrylhydrazyl (62.47%), total flavonoid content (2.44 mg CE/mL), total phenolic content (12.22 mg GAE/mL), total ascorbic acid content (2.53 mg/100 mL) and total lycopene (5.44 ?g/mL) were determined. The ANN model has higher prediction accuracy than RSM. The microstructure, microbiological properties, sensory analysis, ACE (angiotensin-converting-enzyme) inhibitor and antidiabetic effects of the ultrasound-treated tomato vinegar (UTV) (8.9 min and 74.5 amplitude), traditional tomato vinegar (TTV) and pasteurized tomato vinegar (PTV) samples were then evaluated. UTV was generally appreciated by the panelists. It was determined that the microbiological properties were affected by the ultrasound treatment. UTV was found to have more effective ACE inhibitor and antidiabetic properties than other vinegar samples. As a result, the bioactive components of tomato vinegar were enriched with ultrasound treatment and positive effects on health were determined. © 2021 by the authors.
